How to “Declare a Qualifying Event”

 

Qualifying Life Events (QLE) must be completed within 30 days of the event. Qualifying Life Events include, but are not limited to, marriage, divorce, gains or loses coverage elsewhere, birth, etc.

  • Qualifying Life Events through the State Health Benefit Plan must be completed online at the SHBP Enrollment Portal or by contacting SHBP Member Services at 1-800-610-1863. Supporting documentation must be submitted to SHBP within 45 days of plan change.  For additional QLE information please go to the SHBP Life Changes page.
